The candidate requesting speaker status confirms that all information submitted is true and understands the following: The candidate will be held accountable for any false information. Altering program material is a serious violation of PARI's CEU policy. Programs that are given by an unapproved speaker or by someone who has misleadingly classified themselves is not tolerated. Any infringements on PARI's CEU policy will result in termination of speaker status, a recall of any CEU credits issued, and cancellation of any future PARI CEU programs. Access to the PARI CEU website is valid only through 12/31/2012. Upon expiration, the candidate must reapply for a new username and password.
